To All Licensed Child Care Providers (except Family Child Care Home Providers),
The Department of Labor (DOL) has updated its Service Letter form. The revised Service Letter Form is available for download here: Service Letter Form (Fillable PDF). Also, the OCCL website has been updated to provide a link to this new service letter form.
Beginning now or at least by February 1, 2025, please begin using the new service letter form. It is not necessary to resend Service Letters that have already been sent to previous employers unless you wish them to respond using the new form. Please document when you request service letters and in what manner you made the request (US mail, email, etc.) so that you have documentation you met this requirement if the previous employer does not respond. As stated below, DOL can impose penalties when service letters are not requested or returned.
